Meet Carmen and Logan

CARMEN DALTON NP

Carmen Dalton is a Family Nurse Practitioner passionate about health, wellness, and integrative medicine. Her passion for nursing developed at the young age of five. Throughout her life she was always guided to care for others and eventually would go on to graduate with an Associate Degree of Nursing in 2015 from Yavapai College. She then completed a Bachelor of Science in Nursing in 2017 through Chamberlain College of Nursing. In 2022, she graduated from the University of Phoenix with a Master of Science in Nursing as a Family Nurse Practitioner. 
Carmen's nursing career began in alcohol and drug rehabilitation, and has since worked in various healthcare settings, including long-term care, dementia and behavioral health, acute care, medical/surgical and telemetry, progressive and intensive care, hospice, and finally as a primary care provider. While in acute care nursing at Dignity Health YRMC, she  participated in multiple quality assurance projects, mentored new graduate nurses, volunteered for Adopt a Family, the BMHS CNA program, and Relay for life. She sat as a board member for Clinical Ladder, Falls committee, Early Ambulation, Unit Based Council, and was a 2019 Nurse Excellence nominee. 
Carmen is an Arizona native, currently living in Yavapai County with her husband, and their two daughters. She is often found with her nose in a book or working on her own wellness in the gym, but she also thoroughly enjoys hunting, fishing, hiking, and camping outdoors. Carmen's passion for wellness reaches far beyond caring for a patient at the bedside. She aims to teach her patients how to live a quality life. She enjoys learning new concepts and teaching about disease prevention. She believes there is much more we can do to nurture the human body given the tools and knowledge.

Logan Weir CNM

Logan is a Certified Nurse-Midwife born and raised in Arizona. She began her nursing career in labor and delivery in 2019, where she built a strong clinical foundation and developed a deep respect for the physiologic process of birth. She went on to earn her Master of Science in Nursing from Frontier Nursing University, graduating as a Certified Nurse-Midwife in 2025. As a full-scope midwife, Logan provides care across pregnancy, childbirth, postpartum, and the full spectrum of reproductive and women’s health. She also cares for both childbearing and non-childbearing women across the lifespan, including well-woman care, gynecologic concerns, and primary care.

Logan is committed to bridging evidence-based medicine with integrative, holistic practices—creating care that not only treats conditions, but supports the physical, emotional, and physiological experience of each individual. She practices with a focus on physiologic birth, shared decision-making, and individualized, patient-centered care, aiming to ensure every patient feels informed, respected, and empowered in their healthcare choices.

Her path to midwifery was shaped by her own early experience as a young mother. After the birth of her first son at 19, she gained a firsthand understanding of how deeply the quality of care can impact a person’s experience. During that time, she had to travel nearly an hour to access midwifery services—an experience that highlighted the gaps in care that many families face and continues to influence her commitment to improving access within her own community.

Outside of her work, Logan is a mother of five and shares her home with four kitties. She enjoys reading, hiking, and spending time with her family and friends.
